I was wondering the same thing, because I can't find the markers to match the two known die pairs. But you know me, they could be there and I'm just not seeing them. But the doubled rim is just a striking issue right?

Probably but it might have repeated to some extent if it was because of the set up.

It "must be" going to "Bob" this week for his professional attribution! I'm curious to know also Jim if it may be from a different die or just a different die stage.
