I found one that looks almost with two levels of MDD, mostly below the "2" but it's not the best photo:

First.. You need to adjust that font side.. Secondly, you can't post directly into the Error or Variety Forums as it has been this way for a very long time. You must create a new thread in the submission forum.

Another great example can be found for a modern day example of MDD in the Jan/Feb 2008 (Vol 3 #1) page 21 in the Die Variety News. This is an example with the use of Schuler Press.
