The 1991-D DDR is indeed slated for the newest edition of CPG. Ken told me that he's got room for about 75 new varieties in the volume and plans on de-listing several which have been in the book for several editions and are strongly contended as being collectible (for various reasons).
Also coins to watch- Well, I can't give away everything I've worked hard to glean, but you'd do well to grab up the 2006 CONECA Die #3 and #18. Prices will triple [in the right market] when the newest edition hits the presses.

FYI- I'll pay $500 for an example of the 91-D DDR if anyone has one handy. It's going to be a "monster" coin until quantities enter the market. Let me show you that I'm not just fluff- please be in touch if you have any of the above examples available!

Looks to be a form of die damage and not a genuine doubled die.
