Never seen anything like it before. My first thought (and most here will echo) is PSD... but I can't explain Y. (LOL)

Not an error, It's a stamping! Not sure on the Y, It's been done for years, some for advertising, some you just can't explain. Enjoy!:O

It is called a counter stamp. As mentioned, it has been going on forever. Some companies use them as advertising. Here's a good site that lists a lot of Merchant counterstamped coins.

That one looks like a garage job, I have a set of letter stamps out in mine.

Yeah, I see these from time to time. I also have one that has a cut-out in the shape of a cross. It's dated 2000 and yeah, you guessed it, it's has the Wide AM reverse! Go figure?
