What would your first thoughts be if you came across this otherwise mint-state 1999 cent?

Depends on if it was a type 2 reverse.
I have seen this before. Go to http://conecaonline.org/content/recentfinds.htm. Half way down the page. Same "cut out" on a 2000 wide AM.
