I think this is a machine double but I would like second opinion please. Also its off center?

I do believe that it is not off center but a mis-aligned die and with some machine doubling on the reverse.
Just my opinion

I agree with Steven: MAD obverse, not off-center, and (probably) machine (maybe ejection) doubling on the reverse

To put it quickly, MAD is when one die, usually the hammer die, is slightly misaligned but the anvil die is correct.
An off center error occurs when the planchet isn’t centered when struck (I believe) because the collar is jammed
