Judging from the amount of flow lines, I would give a definite yes to die wear. -
It looks like die wear coupled with some mild to strong machine doubling.

I don't understand why you think die wear would only affect part of the die. Or why die abrasion or a struck-through-grease wouldn't affect all of the die.
