Here's a good example, 3 classes (class 4, 5, 8) of hub doubling on the same die:
And any coin can have MD so you could potentially find an example that also has MD.
Added:
Just like any single class doubled die, the same causes can cause multiple classes.
Some dies were hubbed multiple times so a different class of doubling can be there on each hubbing.

Just adding, I think there can also be multiple classes of doubling from the same hubbing.
For example, there can be tilting (class 8) plus pivoting or offset so that could be from the same hubbing.
Not sure how to explain it better, I mean you might have die that was hubbed twice but in the end it might have multiple classes.
Maybe someone can add to that and explain it betterComment

You can find some videos on the internet.
I think TLC or Discovery had a show about the mint and die making but it didn't go into doubled dies, it showed about making of the galvanos for state quarters.
Books like the CPG have sections on die making.Comment
