So, of course I'm hopeing this isn't some kind of vice job either! I'm not getting a good vibe about this one because it is very concaved, and have never heard about a concaved coin with a clash. But I've never seen a concaved vice job either! The reverse is concaved where the extra letters are. The obverse has one small circular depression near the collar which may be related. The extra letters are incuse. I see a D over the T, an E between F and A, and T over the N. Looks to be some more I can't quite make out.

Probably pressed or squeezed lightly between something at some point. The incuse letters could be from a tool or anything with small raised letters.
