I've read a handful of threads pertaining to this medallion / coin / token, and the general consensus seems to sway towards it being a novelty item. I guess Barnes and Noble once released a pirate chest which contained maps, coins, etc..., and many seem to think it came from this little treasure trove, but there has been no final verdict within any of the threads I've come across... It seems to maintain its vague origin and ultimately stumps all who try to identify it... The mystery lives on. Here are a couple of the links I came across:
