These are both on the hammer die. What are the chances it is similar die damage from the die being dropped on the collar die or other object? The process for installing the dies would be the same at both mints so it seems feasible (not very plausible though) that the same "slip and drop" accident could produce similar damage.
(Just throwing that out there onto the wall to see if it sticks!)
